Transition from school
The Transition from School program assists the transition of students with disabilities from mainstream and special education settings into further education, employment and adult life. The transition process can begin from Year 8. Support available through the school-based special education teacher includes:
- Individual transition planning meetings to explore pathways to adult life
- Connect families, students and schools with available services and agencies within the community
- Organise and liaise with local employers for students to have work experiences
- Assist students and families through the transition process by providing information and support

Remote schools pathway grants
